INCIDENTS AND INFRINGEMENTS

St Patrick’s FC requires any significant incident, and ALL red card infringements to be reported to the Club Executive by completing the form below ASAP following the event. Coach, manager and/or involved players may complete the form.

In the case of red cards: ID card must be handed into the referee or placed in the Blue Letter box at the SSFA office on game day.
