St Patrick’s Football Club is committed to ensuring a safe, fair, and enjoyable environment for all players, coaches, officials, and spectators.

Any significant incident occurring during a match or training session must be reported.
All red card infringements must be reported, without exception.
Reports may be submitted by the coach, team manager, or any involved player.
Reports should be completed as soon as possible following the incident.

Complete the official Incident / Infringement Report Form below.
Submit the form immediately after the event to ensure timely review and action.

All reports are reviewed by the Club Executive.
Incidents are assessed in line with SSFA and Football NSW regulations.
Follow-up actions may include disciplinary measures, education, or further investigation where required.

Prompt and accurate reporting supports player safety, fair competition, and SSFA compliance.
Delays or failure to report incidents may impact the club’s ability to act appropriately.
